RE: UCPath Notice: Instructions for 2025 FSA End-of-Year Enrollment
 


 

Please read the following information carefully, as it may relate to you and your benefits.
 

Newly hired employees, newly eligible employees and employees experiencing a qualifying life event with effective dates from October 1, 2025, to November 30, 2025, will not be able to use the self-service portal to make Flexible Spending Account (FSA) enrollment elections or updates for the current plan year. 

Eligible employees may still enroll in FSA or update their current elections by submitting a Benefits eForm corresponding to their qualifying event. Self-Service functionality for FSA enrollment will be available again starting December 1, 2025. Any FSA enrollment submitted through self-service in December 2025 will apply to the 2026 calendar year.
 

The Benefits of Belonging booklet which communicates enrollment instructions will be mailed to employees who become eligible for benefits during the restricted portal period.

How to Make 2025 FSA Plan Elections/Updates

Benefits eForm

Employees can access the Benefits eForm through UCPath by following these steps:

  1. From the main menu, select Benefits & Retirement
  2. Under the Forms category, select Benefits Request eForm
  3. Click Submit under the Benefits eForm Hub
  4. Select the Event Date that corresponds to your benefit event
  5. Select the appropriate Employee Benefit Category
  6. Use the Reason for Request drop-down menu to select the applicable request reason
  7. In the “Please Explain Why You Are Using This Form” textbox, type in FSA Enrollment for 2025
  8. Navigate through the form to make your enrollment updates
  9. Review and agree to the Terms and Conditions and the signature acknowledgment
  10. Once complete, click Submit

 



Submit Enrollment Forms (PDF)
If employees are unable to access UCPath to submit a Benefits eForm, they may request a PDF version of the enrollment forms by visiting UCPath or by submitting an inquiry through Ask UCPath. UCPath will instruct them how to submit completed forms.

The table below shows the current year FSA eForm deadlines and the final 2025 paycheck dates on which UCPath will take FSA deductions:

Note:

FSA coverage is effective the first of the month following enrollment, subject to payroll deadlines. UCPath will deduct the elected FSA contribution amount across the remaining pay periods in 2025. These enrollments do not roll over into the next plan year. To continue coverage, employees must make new FSA elections during Open Enrollment for the 2026 plan year.
